Listed below are all the necessary forms and documents you will need to complete your child's registration at Kaleidoscope Kids Preschool.

Documents #1-10 are for your reference only and do not need to be turned in.

Documents #11-20 need to be filled out and returned to us.

Document #21 is only necessary if you are enrolling in automatic tuition payments.
